Case Management                                                                                                   

Our intake case managers are responsible for completing initial applications for services and gathering the information needed to help determine whether or not a person is eligible for services. To do so, we work closely with individuals and families, often gathering information, with the person’s permission, from a number of other community agencies, including Family CONNECTS, child find teams, Department of Social Services, county school districts , Boards of Cooperative Educational Services (BOCES), North Range Behavioral Health, Vocational Rehabilitation, hospitals, doctors, home health agencies and nursing facilities. An individual must be found to have a qualifying developmental disability or delay before they can access any services or be placed on waiting lists for services.

 

Envision case management services include eligibility determination, waiting list management, authorization and purchase of services and supports, monitoring, and termination and discharge from services. Once a person is found eligible, the case manager will complete an annual plan of services with the person and family/guardian.

 

For a person going onto a waiting list, the plan will tell what services the person needs and when they will need the services. The case manager will make sure that person’s name is placed on the waiting list. Case management services are limited to an annual plan and case management as needed to help with referral and application for services and supports from other agencies, with benefits applicatio ns or with problem-solving.

 

For people who are going to receive services right away, the plan will include what services are needed and why, as well as how the services are going to be provided. The plan may also include a waiting list if other services may be needed in the future. Case management services for people currently receiving services include the annual plan, authorization of services to be provided and ongoing monthly service and support coordination. The case manager will help make sure the person has the necessary evaluations completed and help them find the supports they need. Efforts at interagency collaboration and individual and family empowerment are encouraged, supported and expected.
