As the single entry point for developmental disability support in Weld County, Envision functions as a gateway to a variety of services, including:

To assure that individuals lead the most satisfying lives possible, we listen closely to them before matching their needs and interests to appropriate resources. The people receiving services, legal guardians/families and Envision case managers develop customized annual plans that reflect their goals. Persons with developmental disabilities have a large say in programs, providers and level of guidance.
Our case managers act as the primary liaison between people with developmental disabilities and their service providers. They help with everything from completing applications, gathering information and scheduling evaluations to coordinating services, monitoring progress and authorizing funds.
