Envision’s Adult Case Management provides the single entry point for people requesting determination of eligibility to access developmental disabilities services in Weld County. Determination of an Intellectual or Developmental Disability (I/DD) is required prior to accessing any services through the developmental disabilities system in Colorado. Case Managers work with individuals and families to gather necessary information and to complete intake paperwork for eligibility determination.
For individuals who are also applying for Medicaid Waiver services, including the Home and Community Based Services for Adults with Developmental Disabilities waiver (HCBS-DD) and the Supported Living Services waiver (SLS), Case Managers complete the required long term care assessment with the individual and caregivers. Case Managers are responsible for assuring that all individuals are afforded due process.
For individuals who are also applying for Medicaid Waiver services including the Children’s Extensive Support waiver (CES), and Children’s Habilitative Residential Program waiver (CHRP), Case Managers complete the required assessment with the child and caregivers. Case Managers are responsible for assuring that all individuals are afforded due process.
